Termination of Refractory Tachycardia by External Countershock
نویسنده
چکیده
yENTRICUIAR and supraventricular tachyeardias are occasionally resistant to drug therapy and may culminate in congestive heart failure, circulatory collapse, ventricular fibrillation, and death. In this paper are presented recent experiences with eight patients in whom tachyeardias were unresponsive to drug therapy and were terminated by external electric countershock, usually unider general anesthesia. The technic of external electric countershock was originally developed for the termination of ventricular fibrillation without opening the chest.' It has become established as a safe, effective, and clinically practicable procedure for the emergency resuscitation of patients from this otherwise fatal arrhythmia.2 Using this technic in unconscious subjects, we have also terminated other arrhythmias, namely, ventricular taehyeardia causing Stokes-Adams attacks, and, in the experimental animal, ventricular tachyeardia, supraventricular tachyeardia, and atrial fibrillation.3 These successes in terminating arrhythmias other than ventricular fibrillation suggested the applicability of this technic to desperate, refractory tachyeardias.1-3

متن کاملThe plasma catecholamine response to ventricular tachycardia induction and external countershock during electrophysiologic testing.
Adrenergic activation during electrophysiologic study could potentially alter the electrophysiologic properties of the arrhythmia substrate. However, the catecholamine response to ventricular tachycardia induction and termination during electrophysiologic testing has to date not been quantitated. Therefore, in 13 patients undergoing electrophysiologic study, arterial plasma norepinephrine and e...

Clinically, application of electric currents to the heart has been limited to defibrillation in the operating room when the heart is exposed. In this paper technics are described for the external application of stimulating and countershock currents. They have been used successfully in man to terminate ventricular standstill from any cause and to stop ventricular tachycardia or fibrillation. For...
متن کاملVentricular tachycardia initiated by high energy cardioversion in a patient with an implantable cardioverter defibrillator.
A transvenous implantable cardioverter defibrillator (ICD) was implanted into a 58 year old woman with idiopathic dilated cardiomyopathy who had drug refractory monomorphic ventricular tachycardia (VT). Antitachycardia pacing failed to terminate the VT; termination was attempted at 24 J, which was above the defibrillation threshold. When cardioversion at 24 J was delivered, VT with a different ...
